PROJECT DESCRIPTION
Surgery and hospitalization aim to improve patients medical condition. But at the same time the hospital is also a risky environment. A harmful side effect is the functional decline which is common after hospitalization and major surgery. The reasons for functional decline include firstly a low physical activity level during hospital stay leading to deconditioning and secondly, physiological effects due to surgery and medication. Most surgical patients recover on their own within weeks or sometimes months to the preoperative functional status. But some patients do not succeed in regaining the functional status where they were used to. These mostly older patients have an additional risk for postoperative complications and sometimes protracted or permanent loss of mobility which can impair activities of daily live (ADL) compromising patients independence. These patients are the objective of this research project which will investigate this problem from a preoperative point of view. The focus will be the preoperative functional status of the patient both as predictor and improvement factor of the postoperative course. The statement is: the better the way in, the better the way out.

OBJECTIVES
The object of this PhD-thesis is the role of the various parts of the physical functional status both as predictor and as outcome measure in frail older patients who are scheduled for abdominal and thoracic surgery as major medical treatment.
METHODS
Meta-analysis, observational research, RCT’s and exploratory research.
RESULTS
Determinants of physical functional status revealed to be strongly related with the postoperative course in old patients who had to undergo major abdominal or thoracic surgery. Preoperative therapeutic training of the functional status is feasible in old patients and seemed to positively affect the postoperative course.
